Location

The 0-19 (25 for SEND) team are located in all localities across Northumberland. The service is predominately a home visiting service but contacts can be arranged in Family Hubs, education, colleges and community settings when required. The 0-19 service works closely with all partner organisations who provide services for children, young people and families to provide positive outcomes for all.

Opening Hours

The 0-19 service can be contacted Monday to Friday 8.30am-5pm, excluding bank holidays.

Practitioners can offer some visits and activities up to 8pm weekdays.

Referrals

Referrals into the service for pre school children are through the allocated Health Visitor. SPOC will be able to redirect you to the allocated Health Visitor or support you to speak to another practitioner.

Referrals to the 5-19 service are through a MARF form or by contacting the Early Help Hub on 01670536400 or EHA@northumberland.gov.uk

Office Duty

The 0-19 service offers an Office Duty where a practitioner is available for professionals and service users between the hours of 8.30am – 5pm to discuss any queries if an allocated practitioner is not available.

Health Chat is available for families to access support from the service using the APP, this is a messaging function and is monitored between office duty hours.

The 0-19 Service provides a skill mixed approach to the delivery of care provided. The service includes:

Healthy Child Programme (HCP)

The Healthy Child Programme (HCP) is a public health programme for children, young people and families which focuses on early intervention and prevention. The HCP offers a programme of screening, immunisations, developmental reviews and guidance to support parenting and healthy lifestyle choices.

The Healthy Child Programme contacts in Northumberland include:

  • Antenatal contact
  • New birth visit at 10-14 days
  • Contact at 6-8 weeks
  • Contact at 3-4 months – targeted approach
  • Contact between 9-12 months of age
  • Contact between 2 years and 2.5 years of age

For school aged children the service has 3 Pillars:

Public health pillar:

  • Providing first level advice and support for continence issues
  • Healthy lifestyles support,
  • Sexual health advice,
  • Sleep hygiene
  • Annual school profiles to support key health priorities.
  • Measuring and weighing children in reception and year 6 as part of the National Child Measurement Programme
  • Health promoting activities in the community

Emotional Health and Resilience Pillar:

  • Support school aged children and young people who have been referred into the service with low level, emerging emotional wellbeing needs.
  • Provides evidenced based care packages to both individuals & groups, that focus on identifying and improving areas of need, such as but not limited to – anxiety, low mood, low self-esteem and building resilience.
  • Relax Kids (5-10) and Charge Up (11-16) is delivered in partnership with the family hubs which both focus on improving skills around emotional regulation and building and maintaining positive relationships. The course is delivered over a period of 4 weeks.

Safeguarding Pillar:

  • Complex Case Nurses, Strategy Nurses, Staff Nurse and Child Lived Experience Practitioners working with children and families collaboratively with other partners, focusing on the health needs and capturing the voice of the child.
